  Print out forms, fill in information, and fax to: 334.244.4147
  - Insurance Certificate:
    We do not sell insurance, if you have a small rental you can
     use  your credit card to hold the value of the equipment. 
     Otherwise, you will have to get equipment insurance.
    List insurance as Loss Payee and Additional Insured:
